This package is designed for an individual who has not elected the trust or will package, but nevertheless would like to have powers of attorney in place. This package starts at $500 and includes:

  • 1 Durable Power of Attorney

  • 1 Uniform Power of Attorney

  • 1 Healthcare Power of Attorney

  • 1 HIPPA Authorization

Prices can be negotiated for individual documents.

These documents are available as PDFs. An additional charge will be added for printing.

We do not notarize documents in-house, but we would be happy to arrange a meeting with a notary on your behalf.
